Hello Nurses,
To assist you; I have downloaded the three major forms you need for your transcripts verification by New York state board of Nursing. Find the forms attached below.
Nurse Form 1 – which is for you to fill out All and Notarise it at the end of the form and return to The New York state board by Mail; including making the payment of $143 with it. Address to return to, is on the form.
Nurse Form 2F – which is for you to fill a part of it and sign then send to your School of Nursing to fill their part with your transcripts and return to New York state board of Nursing. Address to return to, is on the form. Your school may charge you for this process.
Nurse form 3F – which is for you to fill and sign your part then send to your Licensing authority to fill their part and verify your License and return to New York state board of Nursing. The address to return to, is at the bottom of the form. Your Nursing Council may charge you for this process.
Please follow instructions carefully. It takes about 6 months to do verification but you can be able to finish it within 2 months if you are SMART about it.
NB* If you are good online; you can check out on New York state board website and fill form 1 online. Make the payment online and then print out the forms 2F and 3F for your school and Licensing authority.
